KRISTIN COBLENTZ SWAN, MSW, MBA, LCSW, LSCSW

Kristin Swan has over 30 years experience in the field of social work. She is a licensed clinical social worker and licensed for independent practice in the State of Missouri and State of Kansas. She is the owner of Swan Therapeutic Services and is a solo practitioner.

Ms. Swan has a commitment to her clients by providing therapy/counseling with in-depth practical approaches for enhancing their lives and changing unhelpful patterns. Clients work with her to not only understand themselves but also to devise practical solutions to their problems. She helps clients discern what is important to them and foster and nurture their efforts toward what matters.

Ms. Swan sees adult client 18 yrs and older only for individual therapy.

Ms. Swan does not see client involved with court with the exception of co-parenting sessions.

In therapy sessions, several different approaches may be utilized such as Cognitive Behavior therapy (CBT), EMDR-Level 2 trained, Mindfulness, Alpha-Stim* and other therapeutic techniques to help clients achieve a client-centered, satisfactory outcome.

Ms. Swan has experience with the following issues: Depression, Anxiety, Mood Disorders, Post Traumatic Stress Disorder, Child Abuse (physical, sexual, etc.), ADHD, Childhood/Behavioral Issues, Life Management Skills, Mood Disorders, First responder needs (police, fire, medical), Parenting Issues, Stress Management, Anger Issues, Grief and Loss, Adoption Transition including Adoption Home Studies, bonding assessments, Divorce/Blended Family Issues, and high conflict co-parenting.

*Alpha-Stim is also offered by Ms. Swan with a doctor's order.

Ms. Swan accepts the following insurances: Alternatives EAP, Aetna, Aetna EAP, Ambetter, BCBS Blue Care, Blue Access, Blue Select, Blue Select ACA, Blue Select Plus, Blue Select Plus ACA, Beacon EAP, Cigna, Cigna EAP, Elevate/Elevate Plus, GEHA Plans: FEHB Standard Option, FEHB High Option, FEHB High Deductible Health Plan, Freedom Network, Freedom Network Select, High Performance Network, IMPACT Solutions, Medica, PAR Traditional, PAS EAP (Preferred Resource Network), Preferred Care, Preferred Care Blue PPO, Preferred Care Blue ACA, United Healthcare, UMR, Optum, Optum EAP, Multiplan and Mercy Managed Behavioral Health.

Effective October 1, 2020 Kristin Swan will no longer be filing claims for UMR insurance if problems arise in obtaining payment. We will try one time. then if not successful we can provide you with a superbill to file yourself.

Effective December 1, 2022 Kristin Swan will no longer be accepting Humana Insurance, Medicaid/MC+ plans and Medicare. I will continue to see those with these insurances until their services conclude.

Effective August 28, 2024 Kristin Swan will no longer accepting Curalinc EAP, Champ VA, Right Solutions EAP, Sandcreek EAP, Evernorth EAP, FEI EAP, Employee Resource Systems EAP, Life Services EAP, Lytle Reach EAP, New Directions Behavioral Health EAP/E4 EAP, Encompass EAP, ACI Specialty Benefits EAP, AllOne Health EAP,

Dear Valued Clients,

I hope this message finds you well. I am writing to inform you of an important update regarding my practice. I have recently partnered with Headway, a mental health platform that provides billing support and client referral services to licensed providers like myself.

What does this mean for you?
There will be no changes to the services you receive from me. I will continue to provide the same level of care and support as always. The primary changes are administrative:

Your Explanation of Benefits (EOB) will now reflect that services are being provided through Headway.
I will begin using Headway’s electronic medical record (EMR) system.
All payments (copays, coinsurance, no-show fees, etc.) will now be handled electronically through Headway, and a valid credit card will be required on file. I will no longer be accepting payments in the office.

I will be gradually transitioning clients over to the Headway system over the next few months. During this transition, you may notice that some services are still billed under Swan Therapeutic Services, while others will be billed through Headway. My goal is to move all clients to the Headway platform once I am fully credentialed with all insurance panels.

To begin, I will be entering clients with Cigna and Aetna insurance into the system starting this week, as I am already credentialed with these providers through Headway.

Once your information has been added to their system, Headway will contact you directly via email or phone to complete intake paperwork. If at any point you need a copy of your medical records, you may reach out to me for records held by Swan Therapeutic Services or to Headway for records created in their system.
